Sports physicals make-up day, July 24

A make-up day for sports physicals for all local athletes in grades 5-12 will be held on Wednesday, July 24 from 9 a.m. to 4 p.m. at Dahl Memorial Clinic. Physicals cost only $25 and payment is due at the time of service.

MHSA pre-participation physical forms are available online at http://www.ekalaka.net by clicking Athletics/Athletic Forms/Pre-Physical Exam Form, or can be found in the front entrance at Dahl Memorial. Be sure to have the form filled out and signed by a parent/guardian prior to bringing it to the clinic. An exam will not be performed if the form is not signed.

The sports physical process at Dahl has changed slightly this year. All physicals done on July 24 will not be billed to insurance. Anyone who still needs a physical and cannot make it to the make-up day on July 24 should call the clinic at 406-775-8738 to schedule an appointment. Scheduled physicals will be charged at the regular clinic visit rate and billed to insurance.
