Changes to patient registration at Dahl

Starting October 1, Dahl Memorial Clinic will be updating its patient registration process. When individuals come for their scheduled appointment, they will need to bring a driver’s license, current insurance card(s) and expected visit copay. Staff will check this information at every clinic appointment to help avoid billing issues.

Please call Dahl Memorial Clinic at 775-8738 for assistance calculating a copay amount prior to an appointment. Staff members will be happy to assist and are thankful to all who are patient during this transition....
