School sports physicals by appointment

If your child missed the scheduled days for sports physicals, don’t worry. Call Dahl Memorial Clinic at 775-8738 to schedule an appointment.

Until school starts in Ekalaka, Dahl Memorial Clinic will be providing sports physicals for a cost of $30. Forms are available online at www.ekalaka.net or in the front entrance at Dahl Memorial. Be sure to have the form filled out and signed by a parent/guardian prior to bringing it to the clinic. The form must be signed by a parent/guardian before a physical can be performed.
